How To Make An Appointment

Appointments are made over the phone. Please call me at (614) 949-6227 to ask any questions or concerns you may have about beginning the counseling process.

I will ask you for some general demographic information, contact information, and a brief description of what you would like to work on in the counseling process.

Once we set up an appointment, you will need to go to the 1st Session Document Packet and print the Clinical History, HIPAA Acknowledgement of Receipt, and the Information & Consent Forms.

Please fill them out and bring them to the 1st session.
